    DescriptionWith the rising number of older individuals in the community, the need for suitable health care has also increased. The hospital Emergency Department (ED) remains the main access point into the health system for our older community members, with visits expected to expand rapidly in the future. However, unless it is an emergency situation, the ED is ill-equipped
    to provide the necessary care for older patients with multifaceted medical histories and needs, which can range from chronic illness and cognitive decline, to complex psychosocial issues. This can result in incorrect diagnoses, longer hospital stays, underutilisation of community services and poorer
    outcomes for patients. Community support may be more suitable, providing ongoing support and enabling the older person to enjoy quality of life.
