    Abstract

    This report provides information about the outcomes of the Cooperative for Healthy Ageing Research and Teaching (CHART) project, which was conducted in the ACT and Southern NSW from July 2012 to February 2015. The main aims of the CHART project were to improve the capability of the aged care workforce, promote innovation in the aged care sector, and thereby enhance ways of meeting the complex care needs of older people. To achieve these aims, the CHART project utilised a collaborative or active engagement model that involved a range of key stakeholders working in partnership, towards common goals.This report describes how these aims and goals were achieved, and also how outcomes of the CHART project will continue to function after the project’s official completion.
