Abstract
Background and Aim/s
Vertical and horizontal abuse (VHA) has been reported in the nursing literature for over thirty years. Previous work has focussed specifically on VHA in nurses working in the industry, while other studies explore VHA in nursing students generally, there has been little attention paid to episodes of VHA which may occur among student nurses learning in simulated environments. To create change and to better inform curricula, the aim of this study was to investigate episodes of VHA which may occur in a simulated laboratory.
Methods
This qualitative critical ethnography framed by Bourdieu’s social practice theory (1977) explores episodes of VHA in nursing students learning in a simulated environment. Ethical clearance was obtained, and student (n=43) and academic participants (n=3) from a second-year undergraduate nursing program were included in the study. Data comprised of observations and interviews, photographs and floor plans of the simulation laboratory. Data analysis was informed by Carspeckens (1996) pragmatic horizon analysis.
Results
Episodes of VHA was evident among second-year nursing students and academics learning and teaching in a simulated environment. This study showed the physical, organization and social space of learning and teaching have been influenced by curriculum design but more significantly the new managerial approaches to university and healthcare organizations. In this context, the pressure from outside the university and the challenge for capital within the university have begun to fracture various fields of practice and in particular the field of the simulation laboratory.
Conclusion
This study calls for both an educational and organizational response to the findings. A curriculum response to reveal and acknowledge - and transform - the use of embedded symbolic violence and an organizational response to acknowledge the driving forces behind the institutional isomorphism of higher education and health care which has manifested in acts of VHA.
