Supporting older people in the community: Project evaluation report 2

This report offers an evaluation of the second phase of the Supporting Older People in the Community project funded by the Murrumbidgee Primary Health Network. The initial phase was completed at the end of June 2021.The second phase immediately followed and was completed at the end of June 2022. The success of phase one was the instigator for the second phase of the program. The project over both phases has resulted in clearly identifiable successes. The positive outcomes from phase one were continued into the second phase of theprogram.
